*the name of the house that you would like to book

*the number of adults/children in your booking party

*if you would like to bring your four-legged doggy friend, including its breed and size (Lakeside and Hilltop Retreat only)

*your preferred dates

*your name and contact details .....phone and/or e-mail

*whether you require a port-a-cot (please bring own baby bedding for cot) and or highchair

*any special requests or questions
